2 Recent Coloured Photograph (Fresh ones i.e. NOT used earlier for any visa) should be:/n35mm x 45mm in dimension/nOn matte finish paper/nWith White background/nWith 80% face coverage, non-smiling [without sunglasses, a hat/cap or other head covering, unless the applicant wears such items because of their religious belief or ethnic background/nnot previously used in a visa

Original Covering letter (addressed to Embassy of the Republic of Azerbaijan)

Approval to be arranged from Ministry of Foreign Affairs.

Original HIV Certificate from any Government recognized Hospital / Medical Centre.

Travel Insurance covering entire stay

Travel Itinerary (ticket) .

Authority letter authorizing Mr Rishipal and Mr. Deepak to submit and collect the documents on applicants behalf. Authorization letter should be DULY NOTARIZED
