Valid passport
with at least Six month validity from the date of travel
Visa Form
5 Forms (to be printed back to back Duly filled - Hand written NOT accepted only Typed)
Passport Size Photos
5 Photograph (Fresh ones i.e. NOT used earlier for any visa)

Visiting card of the passenger.
Personal ITR for last 3 years.

Salary Slips for last 3 months

Rs. 7200/- by cash Handling Charges Applicable NOTE: Visa Fee is to be deposited at the Standard Chartard Bank, Malcha Marg, Chanakya Puri Branch in A/c # 5220 5840 236 favouring High Commission of Pakistan. The bank in turn shall generate a receipt which is to be submitted along with the Visa Application Form.
