    Travel History in the last two months (other than Philippines)- Name of country and Date of departure and arrival with purpose of visit, also share copy of respective visas with immigration stamp pages.

  • Previous Philippines visa copy (In case of previous visit was on the basis of on arrival then share passport`s immigration stamp page copy).
