    Electronic form.

  • Credit Card Copy
    Not mandatory.
  • Original Visiting Card (Optional).

  • Bank Statement
    Original Bank statement of last 6 months Stamped from Bank (Personal).
  • Income Tax Returns
    Last 3 years ITR (Personal).

  • If you are an employee
    Last 3 months salary slips., NOC from Company.
  • If you are proprietor/owner of the company
    Original Bank statement of last 6 months Stamped from Bank (Company). ITR of Last 3 years (Company). Company proof (GST/ Incorporation certificate/Partnership Deed etc).
  • Cover Letter
    On company’s Letterhead.
  • If the applicant is a minor (under 18 years old)
    Original visa letter of consent signed by both parents duly notarized along with a copy of the passports of both parents (or another ID document that bears a signature).

Visa Agent Documents

  • Signed Authority Letter
    (Signed by Applicant) for collection of passport (only in the case where the application was submitted by the applicant and has to be collected by Mondy) – Mandatory.

  • Special Instructions
    Maharashtra, Goa and Karnataka issued passport to be applied in Mumbai only.
  • The Embassy of Brazil
    With Effect From 12-Jan-2015, It has made it mandatory to upload. All Above mentioned documents while filling up the online form, else embassy will not accept any visa application.
  • Original Yellow Fever Certificate
    It is mandatory at the time of travel (Not required for visa processing however).
